About the role

Under the direction of the Behavioral Health leadership, the Behavioral Health Supervisor, is responsible for assisting in the program development and implementation of the care coordination and care management operations in BH Department. The Supervisor is responsible for overseeing daily operations of care management, coordination of care, and continuity of care for KHS members utilizing behavioral health services. In addition, the Supervisor provides oversight and collaboration, including providing workflow, programmatic and practice management consultation. This position engages the care team members, working collaboratively to build and maintain high levels of engagement, as well as excellent member experience.

This position, like all positions within the Behavioral Health Department, is expected to model behavioral health principles of relationship-based care, as well engage in promoting education and understanding of behavioral health and its importance in whole health, to those within KHS and in the community.

Essential Duties and Responsibilities

  • Develop effective relationships with co-workers, direct reports, and community partners, modeling excellent interpersonal skills and collaboration in working toward job duties and goals.
  • Model the importance of member experience and engagement and engage with other leaders in at KHS in developing and implementing member and Team Member experience improvement activities.
  • Communicate, formally and informally, the nature and importance of behavioral health services in overall health to the BH department, as well as the larger organization.
  • Engage with other KHS leaders and community partners in developing and implementing large scale public health education on BH Services.
  • Work closely with the BH leadership to develop effective care coordination and care management principles, processes, and workflows as well as monitor care management services to ensure compliance with all KHS, state, and federal regulatory requirements.
  • Provide leadership, support, and administrative supervision of Team Members, to ensure delivery of high-quality, cost-effective services to members.
  • Proactively recognize problematic or risk areas and help develop effective and efficient solutions in conjunction with BH leadership.
  • Collaborates with BH providers, KHS members and other departments regarding BH Services concerns.
  • Professionally and collaboratively Interface with Utilization Management Department, Member Services, and Care Management Department to resolve issues and establish workflow processes to ensure delivery of high-quality services.
  • Become proficient in all electronic medical management systems to assist in training of new staff members.
  • Professionally represent the department at selected KHS meetings including external events as necessary.
  • Collaborate with BH Supervisors of Network providers on the tracking of HEDIS measures and implement interventions to meet goals.
  • Provide exemplary leadership for all assigned BH team members ensuring team members are given every opportunity to succeed in their positions and are supervised in accordance with Department and Human Resources (HR) policies.
  • Identify deficits within the department and actively find solutions, including but not limited to providing trainings
  • Provide supervision as necessary, consulting on high-risk cases and providing direction to the BH Department.
  • Review and update departmental policies and procedures as necessary.
  • Ability to evaluate current programs for efficiency and program outcomes by analyzing specific data and providing suggested improvements.
  • Refers appropriately to and maximizes use of community services, support programs, internal/external resources available to Members including Case Management, Enhanced Care Management, Community Support Services, Palliative Medicine, Behavioral Health, Transplant programs, pain management, Housing services, Long-Term Services and Supports, etc.
  • Ability to perform comprehensive assessment of patients and create a care plan to address medical, psychosocial, and financial concerns or issues facing the member.
  • Solves complex problems and/or conducts complex analyses impacting transitions/readmissions.
  • Facilitates case conferences to ensure safe transitions of care as Member moves through various stages of treatment.
  • Educates and supports Members to assist them in the best decision making for their health and wellness.
  • Adheres to KHS’s Code of Ethics and Business Conduct and all company policies, e.g., confidentiality, attendance, safety/security, use of equipment and technology, appearance and demeanor.

REQUIREMENTS:

CORE COMPENTENCIES / KNOWLEDGE & SKILL REQUIREMENTS

  • Significant knowledge of integrated clinical and behavioral health care programs and processes in a patient centered medical home.
  • Participates effectively in a leadership role with the multidisciplinary team approach to physical and mental health.
  • Experience in providing and supervising services in a highly diverse work environment to a highly diverse and vulnerable population.
  • Demonstrated leadership ability to create effective teams, provide and communicate vision and effectively manage change.
  • Experience in developing and implementing clinical and behavioral programs.
  • Strong desire to work in a mission driven organization that is committed to reducing health care disparities and providing the highest standard of care to all patients.
  • Ability to remain solution-focused and respectful in all interactions with staff, coworkers, vendors, and clients.

EDUCATION, EXPERIENCE & TRAINING REQUIRED:

Master’s degree from an accredited college or university in Healthcare Administration, Behavioral Health, Sociology, or similar required; AND Two (2) or more years in outpatient medical or behavioral practice management level experience or other relevant healthcare setting.

Clinical license in Social Work, Marriage and Family Therapy, or Behavioral Science required.
