Mt. San Antonio Gardens also known as "The Gardens," is a nationally recognized not-for-profit Life Care retirement community of 400 residents in Southern California, located in Pomona/Claremont, California, and is seeking a caring, capable, self-motivated Family Nurse Practitioner (NP) or certified physician assistant (PA-C), with at least 2 years of experience in clinical ambulatory care, preferably with a desire to care for the geriatric population. The NP or PA-C will be an integral part of our trail-blazing Interdisciplinary Team (IDT), consisting of a supervising physician, LVNs, physical, occupational and speech therapists, clinical dietician, pharmacist, chaplain and social worker. Mt. San Antonio Gardens is dedicated to the development of an innovative healthcare delivery system to improve coordination and reduce the obstacles for the older adults whom we serve.

This position is part time approximately 8 to 10 hours per week and offers a competitive salary of between $50 to $70 per hour. The hours are divided as follows: 6.5 hours every Friday and 1.5 hours every Tuesday from 10am to 11:30am to participate in virtual interdisciplinary team rounds.

Responsibilities:

  • Direct patient care via the Residential Health Services Clinic for residents in independent living, assisted living, and skilled nursing.
  • Room visits as needed if a resident is too ill or debilitated to come to clinic.
  • Conducting Employee physicals for the MSAG staff.
  • Reviewing and signing off on test results and orders.
  • After hours phone call coverage when campus physician is not available or away on vacation.
  • Inbox coverage when campus physician is not available or away on vacation.

Education, Experience, Certifications

  • Must have current license, board certification, and DEA certificate to prescribe scheduled medication
  • At least 2 years of clinical ambulatory care experience- preferably in geriatric care
  • Must be proficient using an electronic medical record - Practice Fusion EMR
  • Ability to work as part of a team
  • Strong administrative and clinical skills
  • Proficiency in Microsoft Office

Malpractice Insurance will be paid or reimbursed annually, if needed.

Ability to maintain effective communication with the community’s residents, family members and diverse staff
Ability to work with the utmost integrity
