Develops and executes strategies for growing Avera@Home service lines: home health, hospice, and Care Transitions through program development and partnering with referral sources. The director will provide strategy for the sales, retention and business growth functions of Avera@Home. The director will develop innovative clinical programming to meet the strategic objectives of Avera and Avera@Home with positive patient experience and outcomes in a population health model.
Supervisory Responsibilities
Responsibilities including interviewing, hiring, developing, training, and retaining employees; planning, assisting, and leading work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems.
Education and/or Experience
Bachelor’s degree in nursing, therapy, social work, nursing home administration or related field is required.
Master’s degree is preferred.
Minimum of five years’ experience in healthcare industry and/or home and community based services is preferred.
Understanding of the health care environment and current healthcare issues required.
The position requires strong leadership, relationship-building, project management, and analytic skills.
Licensure/Certification/Registration
Licensed as a Registered Nurse, Physical Therapist, Occupational Therapist, or Social Work (BSW or MSW) is required.
If nursing prepared - Licensure as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure is required. Licensed as a Registered Nurse in the states of Nebraska, Iowa, Minnesota, North Dakota may be required depending on the service area.
If therapy prepared must hold one of the following - Licensed in the State of South Dakota as an Occupational Therapist required. Licensed as an Occupational Therapist in the states of Nebraska, Iowa, Minnesota, North Dakota may be required depending on the service area. OR Licensed by the State of South Dakota as a Physical Therapist required. Licensed as a Physical Therapist in the states of Nebraska, Iowa, Minnesota, or North Dakota may be required depending on the service area.
If social work prepared - Current license in Social Work in the State of South Dakota required. Current Social Work license in the state of Nebraska, Iowa, Minnesota, or North Dakota may be required depending on the service area.
